THE ROLE OF IVABRADINE IN THE EFFECTIVE TREATMENT OF POSTURAL ORTHOSTATIC TACHYCARDIA ARTER «COVID-19» (literature review)

Introduction:     COVID-19 is a novel type of coronavirus that has had a profound impact on global human health and economic systems. Since its emergence in 2019, it has escalated into a worldwide pandemic, resulting in significant mortality across populations. The COVID-19 pandemic not only led to a spike in acute-phase illnesses and deaths, but also, over time, contributed to the development of so-called “long COVID” or chronic post-COVID symptoms in many patients over time.

One manifestation of post-COVID syndrome is Postural Orthostatic Tachycardia Syndrome (POTS). This article presents a literature review focusing on the pathogenetic mechanism of action and the clinical effectiveness of ivabradine, an If-channel inhibitor, in treating tachycardia — one of the most common symptoms of post-COVID syndrome.

The purpose of the study: To conduct a systematic review of recent studies assessing the efficacy of ivabradine in the treatment of post-COVID-related tachycardia and other cardiac symptoms, considering their increasing prevalence.

Materials and Methods: A comprehensive systematic review and meta-analysis was conducted to explore the relationship between post-COVID POTS and ivabradine therapy. Relevant full-text scientific articles were obtained from the following academic databases: PubMed, MEDLINE, Embase, Cochrane Library, Mendeley, Web of Science, and Scopus.

Results and discussion: Ivabradine demonstrated high therapeutic effectiveness in managing POTS by reducing heart rate and protecting end organs. This review includes clinical examples of ivabradine administration in patients who recovered from COVID-19, and evaluates studies suggesting the potential future use of ivabradine in treating a broad spectrum of post-COVID clinical manifestations.

Conclusions: Ivabradine offers an effective approach for managing postural orthostatic tachycardia and plays a significant role in the treatment of patients with other post-COVID cardiac complications.
